ePortfolio Workshop Request Form for ACE Program

Dear Faculty Member,

Thank you so much for your support of the ePortfolio Project at LaGuardia.  We are excited to assist you and your students in the creation of ePortfolios for your course. To help us meet your needs, please complete the ePortfolio Workshop Request Form below at least 2 weeks in advance.

ePortfolio Workshop will include the following:
a. ePortfolio Introductory Presentation
b. ePortfolio Resources and Tutorials
c. Step-by-step demonstration of creating an ePortfolio in Digication
d. Hands-on-activity of creating your own ePortfolio

Please provide the following information :
1.- Enter the program name. (Example: CLIP / VUB / PDA)
 
2.- In what class will you be using ePortfolio this semester? Enter the class name in Digication.
(Example: CLIP_FA12_MAR1)
 Note: You need to fill out this form for each course.
3.- Approximate number of students enrolled in your course:
 Note: Please contact Emad Nassar (ACE Help Desk Administrator phone: 718-482-5133 E-Mail: Enassar@lagcc.cuny.edu ) for ACE students' Digication accounts.

4.- Do you already have an STM who regularly meets with you and your students in a computer lab?
